If you wish to talk to a health professional, please contact the receptionist
who will ensure the message is passed to the relevant clinician. GPs usually
return calls when surgery finishes. Please advise the receptionist if your call
is urgent.

Obtain My Test Results?
Please leave three working days before phoning for blood test results. X-rays
can take up to four weeks. Please ring after 2.00pm for test results. Results
will only be given to the person who had the test unless that person lodges prior
written consent for the practice to release results to a proxy.

Request A Home Visit?
Patients are requested to telephone before 10.30am if a visit is required that
day. Emergency visits only will be arranged after that time. Please give the
receptionist as much information as possible to enable the doctor to allocate
priority to house calls. Please remember that several patients can be seen at
the surgery in the time it takes to do a home visit.

Make An Appointment?
Surgeries run from 8.30 - 11.30am and 4.00 - 6.00pm.
Routine appointments are available from 8.30 - 11.00am and 3.30 - 5.30pm.
Please contact the surgery to make an appointment with a health professional
either by telephone or calling into the surgery.
Same-day appointments for minor illnesses (eg coughs, colds, earaches, routine
medication review), telephone advice and appointments may be obtained from the
practice nurse (please phone reception).
For appointments with the nurse, please book your appointment in advance. Please
note, nursing staff can deal with most of the following:
• Blood Pressure
• Asthma Checks and Diabetic Checks
• Baby Immunisations
• Injections
• Coughs, Colds and Sore Throats
• Contraception
• Hay Fever
• Travel Advice
• Simple Medication Review
• Earache
• Eczema

Obtain a repeat prescription?
Repeat prescription requests are not taken over the telephone, for medico-legal reasons, as errors can occur in verbal communication.

Please do not ask the doctor to issue repeat prescriptions during a routine consultation; it may only take an extra minute, but over the course of the day it adds up to an extra five spare appointments. |

The prescription can be send to your home address, but please remmeber to enclose a stamped, addressed envelope. It can also be collected from the surgery |

HOW
DO I...?
Register With The Practice?
If you wish to register with the practice we will require proof of your address.
This is because registration is based solely on residency within our catchment
area. Please provide any of the following: a TV Licence, bank statement, utility
bill, documentation from Basingstoke & Deane Borough Council or a housing
association/landlord. The document should show your name and address. You will
then be given a simple New Patient Questionnaire and a Registration Form to complete.
Your medical records will be obtained from your previous GP by the Primary Care
Trust's Registration Department and forwarded to the Bermuda Practice as soon
as possible.
