New Patient Registration
If you wish to register with the practice we will require proof of your address. This is because registration is based solely on residency within our catchment area. Please provide any of the following: a TV licence, bank statement, utility bill, documentation from Basingstoke & Deane Borough Council or a housing association/landlord. (The document should show your name and address.)
You will then be given a simple New Patient Questionnaire and a Registration Form to complete. Your medical records will be obtained from your previous GP by the Primary Care Trust's Registration Department and forwarded to the Bermuda Practice as soon as possible.
